Utah EVS Guide
You’ve made an appointment with a Qualified Medical Provider (QMP) for medical cannabis and you now need to do the state application.
You hear acronyms like UtahID and EVS. But what are they and how do you complete the state application?
In this Utah EVS Guide, we explain everything to help you get ready for your appointment.

What is UtahID?
UtahID is Utah’s online portal that gives Utah residents access to all the state’s services from one place. Think of it as a digital passport that connects you to all of Utah’s social services.
To apply for a medical cannabis card in Utah and before getting started with EVS, you MUST have a UtahID account.
Additionally, UtahID and EVS are synced to work together, meaning that logging into your UtahID account automatically logs you in your EVS account.
What is EVS?
EVS, or ‘Electronic Verification System‘, is Utah state’s online portal for patients to apply for a medical cannabis card.
In it, you can see your pharmacy purchase history, your QMP’s recommendations, as well as make application payments.
It is also used by qualified medical providers to issue patient medical cannabis card certifications and manage patients’ dosing recommendations.
Basically, it is used for all things medical cannabis in Utah so that the state can keep a record of it.

Getting Started!
Before moving forward in this process, you’ll need a few pieces of information on hand, including:
- Social Security Number
- Valid Government-Issued ID
- Email Address
- Date of Birth
Create a UtahID Account
- Go to UtahID
- Enter a valid email address and click ‘Submit‘
- On the next screen, enter the code that was just sent to the email address you previously entered
- Enter your information on the next screen
- Click ‘Submit“
- Create a new password
- Click ‘Submit‘
Store your password in a safe place! You will need to access your UtahID account throughout your medical cannabis journey, so don’t lose it!
- Enter your the email address and password you used to create the account and click ‘Submit‘.
You will now be redirected to create a new account in EVS.

Note: UtahID and EVS are synced to work together, meaning that logging into your UtahID account automatically logs you into your EVS account.
Register & Apply in EVS
It is essential that the information you enter in EVS accurately reflects what is on your government-issued ID.
Anything different will cause errors (and headaches) throughout your application process.
Utah EVS Registration
- Go to evs.utah.gov
Note: If you’re not already logged into your UtahID account, you will be prompted to do so now, after which you will be redirected to the EVS registration page
- Click ‘Patient Registration’
- Enter your information
Warning: Your name must be entered EXACTLY as it appears on your government-issued ID.
- Click the ‘Click Here‘ text link to access your EVS home page.
Your EVS registration is complete!
The last step is to complete your EVS application.
Utah EVS Application
It is essential that the information you enter in EVS accurately reflects what is on your government-issued ID.
Anything different will cause errors (and headaches) throughout your application process.
Let’s get started.
- From your EVS homepage, click ‘Tracking Inbox‘
- Click the ‘+ New‘ button
- Fill in your information
Enter your name exactly as it appears on your government-issued ID. Anything different will cause issues with your application.
All fields with a red ‘R’ must be filled in.
- Check the ‘Patient Acknowledge‘ box
- Click Save
Enter your name exactly as it appears on your government-issued ID. Anything different will cause issues with your application.
The page will refresh an you’ll notice 2 changes:
- A progress bar, with a large, green dot above the word ‘Draft‘ and
- A set of gray tabs
- Click the blue ‘Save & Submit for Certification‘ button at the bottom of the page
The page should refresh and there should now be a large-green dot above the word ‘Awaiting Certification‘ (see below).
