Once your QMP has certified you for your medical cannabis card, you will need to complete your state application by paying the $15 application in EVS.
Below are the basic steps showing you how to pay the EVS application fee.
- Login into your EVS account
- Hover over Tracking Inbox and click Patient (NOT New Patient)
- Click the Payment tab
- Click the +New button
- Click the Click Here to Pay button
- Complete the payment form* in the popup tab that appears
- Your done!
Detailed Instructions
- From the EVS homepage, hover over ‘Tracking Inbox‘ and click ‘Patient‘ (NOT ‘New Patient)
Note: The only reason you will ever need to click ‘New Patient’ is if you are applying for a minor
- Hover your mouse over the payment tab and click ‘New Payment‘
- Click the red button ‘Click Here to Pay‘
- Fill in all the fields on the payment form page
- Scroll down and click ‘Continue‘
- Ensure the information you’ve inputted is correct. Click ‘Yes‘
- On the receipt page, click ‘Continue‘ to be taken pack to the Payment tab in EVS.
You will notice now that your status is in ‘Active Conditional‘.
You can now print your card from within EVS. Just navigate back to your application page (see step 1 & 2).
Once inside your application page (make sure you’re on the Patient tab), click the blue button titled Print Card’ to access it.
Congratulations! You are now a Utah Medical cardholder and are ready to purchase your medical cannabis from your pharmacy!
