New Patient Guide
Getting your Utah Medical Cannabis Card is a fairly simple process, but there are several things you’ll need to take into consideration.
This step-by-step tutorial will guide you from beginning to end on how to get a medical cannabis card in Utah.
Let’s get started.
Step 1: Identify Your Qualifying Condition
This is the first and most important step. Without a qualifying condition, you may be wasting your time with the rest.
So what are the qualifying conditions? Below we’ve listed the qualifying conditions in order of most common to least common:
- Chronic Pain
- PTSD
- Cancer
- Persistent Nausea
- MS & Muscle Spasms
- Crohn’s Disease & Ulcerative
- Colitis
- Epilepsy & Seizures
- Autism
- HIV & AIDS
- Rare Conditions
- Terminal Illness
- Hospice
- Cachexia
- Alzheimer’s
- ALS
Have another condition not on this list? Don’t worry!
Utah state has defined chronic pain as any “pain lasting longer than 2 weeks,” which means that any condition or diagnosis that is associated with chronic pain may qualify you.

Pro Tip
It is likely your QMP will want to verify any diagnosis. The best accepted form of verification is a medical document from a medical professional familiar with your condition.
Step 2: Create a UtahID Account
- Date of birth
- Social security number
- Valid government issued ID
- Email address
- Go to the UtahID Account Creation page
- Enter a valid email address and click ‘Submit‘
- On the next screen, enter the code that was just sent to the email address you previously entered
- Enter your information on the next screen
- Click ‘Submit“
- Create a new password
- Click ‘Submit‘
Store your password in a safe place! You will need to access your UtahID account throughout your medical cannabis journey, so don’t lose it!
- Enter your the email address and password you used to create the account and click ‘Submit‘.
You will now be redirected to create a new account in EVS.
If you are not redirected, click the following link: evs.utah.gov
Note: UtahID and EVS are synced to work together, meaning that logging into your UtahID account automatically logs you into your EVS account.
Step 3: Register & Apply in EVS
It is essential that the information you enter in EVS accurately reflects what is on your government-issued ID.
Anything different will cause errors (and headaches) throughout your application process.
Let’s get started.
EVS Registration
- Go to evs.utah.gov
Note: If you’re not already logged into your UtahID account, you will be prompted to do so now, after which you will be redirected to the EVS registration page
- Click ‘Patient Registration’
- Enter your information
Warning: Your name must be entered EXACTLY as it appears on your government-issued ID.
- Click the ‘Click Here‘ text link to access your EVS home page.
Your EVS registration is complete!
The last step is to complete your EVS application.
EVS Application
It is essential that the information you enter in EVS accurately reflects what is on your government-issued ID.
Anything different will cause errors (and headaches) throughout your application process.
Let’s get started.
- From your EVS homepage, click ‘Tracking Inbox‘
- Click the ‘+ New‘ button
- Fill in your information
Enter your name exactly as it appears on your government-issued ID. Anything different will cause issues with your application.
All fields with a red ‘R’ must be filled in.
- Check the ‘Patient Acknowledge‘ box
- Click Save

The page will refresh an you’ll notice 2 changes:
- A progress bar, with a large, green dot above the word ‘Draft‘ and
- A set of gray tabs
- Click the blue ‘Save & Submit for Certification‘ button at the bottom of the page
The page should refresh and there should now be a large-green dot above the word ‘Awaiting Certification‘ (see below).
You are ready to be seen by a QMP to be certified for your Utah Medical Cannabis Card.
Step 4: Visit Your QMP
At this point, you will need to schedule a visit with your QMP, if you have yet to do so.
Your QMP will evaluate you to see if medical cannabis is working is a good option for you, as well as discuss a treatment plan that works best for you.
Note: The more details you give your QMP about your condition the better. This will allow them to make the best recommendation for you and how medical cannabis may help you.
Step 5: Pay the EVS Fee
EVS will notify you once you have been certified by your QMP. When that happens, you will need to pay the EVS fee before you can access your card.
Note: You will not be able to pay the EVS until your QMP has certified you.
- From the EVS homepage, hover over ‘Tracking Inbox’ and click ‘Patient’ (NOT ‘New Patient’)
Note: The only reason you will ever need to click ‘New Patient’ ever again is if you are applying for minor.
- Hover your mouse over the payment tab and click ‘New Payment‘
- Click ‘Click Here to Pay‘
- Fill in all the fields
- Scroll down and click ‘Continue‘
- Ensure the information you’ve inputted is correct. Click ‘Yes‘
- On the receipt page, click ‘Continue‘ to be taken pack to the Payment Page in EVS
Step 6: Access Your Card
Once your application has been approved for your application, the state will notify you via email with directions on how to access & print your card. We will detail the process here as well.
- Log into EVS and click your name
- Click the blue ‘Print Card‘ button
- Follow your computer’s prompts to print your card.
Pro Tip
A digital copy of your card is acceptable to show at any medical marijuana pharmacy. Many are also willing to print it out for you.
